Major Project Consent Forms

Here is a blank consent form. You can print it off yourself, or download it and email it to people to fill in before you film them. Or there will be physical copies at the front of the newsroom. I suggest you grab a bunch and keep them with you. I think it’s best if you get people to fill in the form with you standing next to them, in case they have any questions.

You need a consent form for everyone you interview for your major project documentary — radio or TV. (You don’t need a consent form for vox pops.)

If you are interviewing a child/young person it is the same form. You ask the young person’s parent to fill the form in. (You must discuss interviewing young people with your supervisor.)

When it is time for you submit, you include all the relevant consent forms in your production file. Scan them or photograph them. I will put a separate post up about your production file in a few weeks.

Underneath all the legal language, people are saying that they agree to be filmed/interviewed/edited and they understand that the finished documentary may be seen/heard by the public (it is a piece of journalism after all).

Consent forms are there to protect you and protect JOMEC. If you have a completed consent form, nobody can complain about being part of your film. You should get a consent form from contributors even if they are filmed anonymously. (Remember, though that you should not offer people anonymity without discussing this first with your supervisor. We only give people anonymity in specific circumstances.)
